Post subject: Physical Examination Record for Foreigner

Hi. Im awaiting my work permit from china, and am looking at the documentation i need to fill in. One of which is the Physical Examination Record for Foreigner. It seems to require me to have a complete medical, with lots of tests etc. I am from the UK and i doubt they will do a medical for this reason. Has anyone been through this and can advise further? Thanks in advance

Yep, to be able to apply for your Z-visa at the Chinese embassy you do indeed need to fill out this form.

Since you have to do another health check in China anyway I would try to keep the costs as low as possible.
We completed the form ourself, went to our family doctor who signed and stamped it (use a RED stamp!). The only blood test we did was the AIDS test. (And I have to say that the signature from our family doctor + the AIDS test in Europe already costed MORE than the whole health check in China!! Rolling Eyes )

Husband got his Z-visa without problem.

You shouldn't have to do a Physical before you come here, unless it's a requirement from your employer.

You MUST do it here in the official Health Check place in HongQiao.

That's wrong.

Last year in May the Chinese embassy in Berlin would not issue the Z-visa WITHOUT the above mentioned health check + a negative Aids test. It was a new requirement then. Before they only wanted the Aids test.

(Of course we could have come to China before, do the wonderful health check in China, return to our homecountry and then apply for Z-visa)

Kirishima, you should call the Chinese embassy and ask them whether they need the health check for the z-visa or not. (Maybe it's different for British citzens or it was only an "olympic" requirement last year Wink )

You won't be able to get a free full medical in the UK - it'll cost over £250 I reckon at most places. You'd then need to get the results notarised to make them legal in China. Don't bother - just do it here. All of us on Z documentation have probably had the health check here, including me. When I did it, seemed a pretty clean and efficient process here in Shanghai (and the nurses aren't bad looking either) but you are herded about like an animal a bit. Shouldn't take any longer than an hour to 90 minutes on a busy day.

Just read the Chinese embassy in UK site, seemed like there was no such requirement. Call and ask, if they tell you you need to do it then you obviously have to. But you will still have to do it again here in China as many have pointed out.

I had to do it in Sweden first (about 2000 kuai) and then in Shanghai (around 400-450 kuai... but I think usually it is around 700 kuai)

I'm curious, did they accept your chest x-ray in China?
I tried to give them an old one but they didn't "eat it" since there was no date on the x-ray Laughing

I had the physical done in the UK at my local GP's once 10 years ago. GP looked gobsmacked at the questions and just ticked no on all of them - "e.g. manic psychosis". Sent me off for the HIV test at the local hospital and stapled the results on the form, and stamped it. Chinese immigration people thought it looked official, so didn't care too much about it, and gave me the residence permit. That was 10 years ago though.........

Medea: Only got a report from the hospital in Sweden, not the real pic. Had to do everything in China. At the time I was pissed as hell that they didn't really recognize anything from Sweden (on the foreigner's health examination form). But we all have our experiences with bureaucracy and stupidity here in China, best way is to just let it go and move on.

Applying for a Z visa.

The application form for the Health Check here (amongst other questions) asks something like:

"Any surgical procedures ? If so, provide details and dates"

So, I ticked yes. Recorded Tonsillectomy. Vasectomy. Appendectomy & Hemicolectomy (removal of a segement of small and large intestine - that was a serious procedure with lovely morphine & pethedine = demerol afterward for 10 days).

Went back to the health check place for Foreigners in Suzhou to fetch the report 3 days later as advised.

Surgical procedures : "None". I was astonished, but hey, this is China.

I love the question "Any History of mania or Mental Illness ?"

As if you'd tick YES. I'm sure they could easily verify the truth by bothering to contact Central Medical Records in your home country. Yeah, right.

Same as the health check form you fill out in flight before arriving in Shanghai on the plane.
