I just brought my dogs back to the U.S. from Beijing. Here's what I had to do:
1. I'm not sure what country you are returning to but you will want to check with the agency that handles importation of Pets. In the U.S. it's the USDA and specifically APHIS. For the U.S., the only requirement is that the rabies shot is up to date.
2. There's typically an Animal Hospital that handles all the pets that travel internationally in and out of the city. You will need to bring your dog to be examined. They will give you some paperwork that you will need to bring to the Entry-Exit Inspection and Quarantine Ministry. They should provide an address of the office for you.
3. Bring all the paperwork to the Ministry, pay a couple hundred RMB and they will provide an Animal Health Certificate that certifies the health of your pet and the time of his/hers last rabies shot.
4. You will need to do all of this within 10 days of your travel date as the certificates are only valid for 10-14 days.
5. Make sure the airline is aware that you are traveling with a pet. If it's a kennel and you will put the dog in cargo you will need to reserve the space. They will also have some guidelines to follow (kennel, food, water etc.) They are actually the ones that review a lot of the paperwork. I flew United and they were extremely helpful at the airport with getting our dogs checked in.

You need to take him to the government vet for the rabies vaccine, even if he has had it before. You are supposed to do this at least 30 days before departure.
Within 7 days of departure you can then take him to receive Health Exam after which they will issue the Health Certificate. I believe microchip is also required.
I just went through a similiar process with my cat and received vaccine, microchip and Health Certificate for about 450 rmb.

You may also need an Export Permit as I've read this but I am not sure. |
